We are seeking a dedicated full-time Palliative Care Nurse Practitioner (NP) to join our team in Oak Creek, WI, and surrounding areas.
Our hospice focuses on providing personalized, compassionate care to make patients' final days comfortable and dignified, treating everyone like family.
The NP will develop and manage our palliative care program, handle patient caseloads, and assist with diagnosis, treatment, medication management, assessments, and consultations. Travel to patients' homes or facilities is required.
Candidates must hold a valid APRN and RN license, have or be able to obtain an NPI number, and possess a controlled substance license and DEA number (preferred). 1-2 years of nursing experience, compassion, and integrity are essential.
