Master Social Worker

Seeking a full-time Social Worker with a Master's degree in Social Work from an accredited program and at least one year of healthcare experience.

• Provide social services within a Home Health Care Program, supporting patient and family coping, dignity, and personal worth.
• Assess social and emotional needs, develop care plans, and assist in preparing patients for changes in living situations.
• Collaborate with the healthcare team, evaluate patient progress, and document visits and interventions timely.
• Aid families in understanding medical recommendations and utilizing community resources for optimal support.
• Participate in staff training, quality assurance, and develop or assist in care planning and in-service education.
