Job Summary
Identity Outreach LLC is seeking a Community-based Family Case Manager to provide direct services to children and families through community-based and DCS-related programs. This role includes supervising visits, supporting parents, providing tutoring, and assisting families with skill-building and resource connection in home and community settings.

Responsibilities
- Provide services in home and community settings
- Supervise and document parent-child visits
- Support parents with hands-on skill building (routine, structure, communication)
- Assist with daily living skills and home management
- Provide tutoring and academic support as needed
- Engage fathers and support family relationships
- Connect families to community resources
- Complete documentation (case notes, reports, court summaries)
- Attend team meetings and court (as needed)
Qualifications
- Minimum of an Associate’s Degree in Social Work, Human Services, Psychology, Education, or a related field
- OR
- High School Diploma or GED with 4 years of verifiable experience working with children and familiesPreferred:
- Bachelor’s or Master’s degree in a related field
- Experience with DCS or community-based services
Additional Requirements
- Valid driver’s license and reliable transportation
- Ability to pass background checks (criminal history, CPS, DCS)
- Strong communication and documentation skills
- Ability to work independently in the field
- Flexible availability (evenings and weekends as needed)
Pay: $35.00 - $65.00 per hour
