Occupational Therapist - SNF/LTC

Key Responsibilities- Complete functional assessments and develop individualized treatment plans for residents
- Provide therapeutic interventions focused on activities of daily living (ADLs), mobility, and safety
- Communicate resident progress and recommendations with the interdisciplinary team and families
- Collaborate with COTAs and Rehab Aides to ensure consistent, high-quality care
- Foster a positive, resident-centered rehabilitation environment
- Degree from an accredited Occupational Therapy program
- Active Oregon Occupational Therapy license or eligibility to obtain
- CPR certification or ability to obtain prior to start
- Strong communication, clinical reasoning, and teamwork skills
- Experience in a skilled nursing facility (SNF), post-acute, or long-term care setting
- Familiarity with Nethealth documentation software
- Work in a dynamic, patient-centered environment with a supportive interdisciplinary team
