POSITION OVERVIEW?
A Care Manager for our Mobile Crisis Response Team will provide direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative triage, assessment, planning, and communication process. The position requires traveling within Hillsborough County in a personal vehicle or agency vehicle to facilitate in-home client services in various socioeconomic neighborhoods.

JOB REQUIREMENTS:
- Must have a Bachelor's Degree in Human Services or a related field of study in Human Services, Behavioral Health, or Psychology.
- Must be able to lift and carry up to 50lbs of dead weight
- Must have excellent interpersonal skills
- Minimum of 21 years old or older, possess a valid Florida Driver's license, reliable personal transportation, and insurance.
POSITION RESPONSIBILITIES
- Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
- Complete all clinical and administrative documentation based on established timelines as per program requirements.
- Communicate effectively and maintain cooperative relationships with internal and external resources.
- Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements.
- Maintain efficient workflow management and organization as per program requirements.
- Maintain proficient quality of clinical documentation as per program requirements.
- Other duties as assigned or needed as per program requirements.
WORK ENVIRONMENT:
Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe, supportive environment.
This is a 60-bed Adult Crisis inpatient care facility.
Office setting within the Crisis Center ?
Must be capable of lifting 50 pounds of dead weight alone.
