Qualifications
Master’s Degree in Speech Language Pathology from an accredited program.
Certificate of Clinical Competency or eligibility for a Clinical Fellowship Year.
Assumes responsibility for ongoing continuing education and professional development.
Essential Job Function
- Provides speech therapy services to residents and initiates referrals when appropriate and in accordance with the Therapy Department Procedures.
- Conducts screening of residents at regular intervals to determine need for intervention / treatment.
- Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
- Interprets and communicates evaluation findings to residents, families, and team members.
- Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
- Coordinate’s treatment plan with resident, family and other team members.
- Implements and conducts treatment as outlined in treatment plan.
- Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
- Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
- Documents results of resident’s assessment, treatment, follow-up, and termination of services.
- Adapts treatment to meet the individual needs of the resident.
- Provides, recommends, fabricates adaptive devices, or other equipment. Trains resident, family, and appropriate staff in the use or application of orthotics and prosthetics.
- Instructs resident, family, and staff in appropriate speech-language procedures or home program to allow carry over of therapeutic remediation and goals in daily activities.
- Participates in discharge planning.
- Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using pre-determined criteria.
- Participates in resident, family, and staff education.
- Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
- Adheres to Speech Therapy Department procedures regarding documentation of speech therapy services.
- Provides in-service education to the members of the resident’s care team and education to the community.
- Provides treatment in accordance with established standards of practice, department procedures, and productivity standards.
- Consistently demonstrates sound judgement in the evaluation, planning, implementation, and follow-up of resident therapy programs.
- Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
- Assists with and participates in Quality Assurance Program as required.
- Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are incompliance with applicable regulations.
- Attends and participates in scheduled trainings, educational classes and meetings required for requested for position.
- Ensures a safe environment and utilizes tools and equipment in a safe manner complying with a all infection control, universal precautions and OSHA standards for the healthcare personnel.
- Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/ appropriate personnel immediately.
- Responds to and acts appropriately in emergency or disaster situations.
- Adheres to high standards of cleanliness, grooming, hygiene and dress code.
- Adheres to all policies of company including job description, certification requirements, Mission Statement, HIPPA, confidentiality standards and residents rights.
- Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments.
- Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control
- Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
