Essential Job Functions

1. Provides Occupational Therapy services to residents and initiates referrals when appropriate and in accordance and in accordance with Occupational Therapy Department Procedures.
2. Conducts screenings of residents at regular intervals to determine need for intervention/treatment.
3. Evaluates residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated.
4. Interprets and communicates findings to residents, families and team members
5. Establishes individual resident schedules in conjunction with other team members.
6. Develops treatment plans by identifying a target list, including short- term and long-term goals and methods to achieve identified goals.
7. Coordinates treatment plan with resident, family and other team members.
8. Implements and conducts treatment as outlined in treatment plan.
9. Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance thin physician orders.
10.  Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
11. Documents results of resident’s assessment, treatment, follow-up and termination of services.
12. Adapts treatment to meet the individual need of the resident.
13. Provides, recommends and fabricates adaptive devices or other equipment. Trains resident, family and appropriate staff in the use or application of such equipment.
14. Participates in discharge planning.
15. Supervises certifies occupational Therapy Assistants (COTA) in the implementation of the treatment plan.
16. Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria
17. Participates in resident, family and staff education.
18. Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
19. Adheres to Occupational Therapy Department procedures regarding documentation of occupational services.
20. Provides in-service education to the members of the residents care team and education to the family.
21. Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
22.  Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
23. Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
24. Assists with and participates in Quality Assurance Programs as required
25. Maintains a current knowledge of the law and regulations regarding rehabilitative services and consults with the supervisor to ensure that all employment actions are in compliance with applicable applications.
26. Attends and participates in scheduled trainings, educational classes and meetings required for requested for position.
27. Ensures a safe environment and utilizes tools and equipment in a safe manner complying with a all infection control, universal precautions and OSHA standards for the healthcare personnel.
28. Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/ appropriate personnel immediately.
29. Responds to and acts appropriately in emergency or disaster situations.
30. Adheres to high standards of cleanliness, grooming, hygiene and dress code.
31. Adheres to all policies of company including job description, certification requirements, Mission Statement, HIPPA, confidentiality standards and residents rights.
32. Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments.
33. Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control
34. Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
35. Performs all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization
