Essential Job Functions
- Maintains a clinical caseload
- Responsible for recruiting and retaining all rehabilitation service personnel
- Ensures effective orientation and on-the-job training for all rehabilitation service personnel
- Promotes positive employee relations by facilitating an atmosphere of open commination and problem solving.
- Responsible for conducting performance appraisals of all rehabilitation service personnel
- Responsible for all aspects of operations of rehabilitation service at facility level
- Monitors personnel compliance with standards taking corrective action as needed.
- Works with finance personnel in reconciling billing to ensure accuracy.
- Assures that all program standards are met, including CARF, SNF, and JCAHO standards.
- Identifies program development needs of the community and implements appropriate programs.
- Develops clinical affiliation programs with educational institutions.
- Investigates, follows up and assists in the resolution of staff, service and resident-related issues requested by management.
- Reviews the quality and appropriateness of total services delivered.
- Assesses resources and supplies available in community.
- Tracks MDS Assessment Period Data to include number of disciplines treating, treatment days/weeks and a daily treatment time. Communicates data to appropriate personnel. Coordinates change in MDS data according to community procedure.
- Responsible for utilizing tracking systems and for implementing changes that may be indicated. Ensures compliance with data entry for PPS resource utilization and CPT coding daily. Monitors treatment minutes daily.
- Generates special reports/data as needed to assist Hyatt Family Facilities in assessing the efficacy and cost effectiveness of the Rehabilitation services for the facility.
- Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
- Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
- Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned.
- Assists with and participates in Quality Assurance program as required.
- Maintains a current knowledge of the law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.
- Attends and participates in scheduled trainings, educational classes and meetings required or requested for position.
- Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions and OSHA standards for the healthcare professional.
- Reports all hazardous conditions unclosing incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
- Responds to and acts appropriately in emergency or disaster situations. 28. Adheres to high standards of cleanliness, grooming, hygiene and dress code.
- Adheres to all policies of company, unclosing job description, certification and requirements, mission statement, HIPPA, Confidentiality standards and residents rights.
- Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment.
- Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
- Performs all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interest of the organization.
Physical Demands of the Job
- Frequent tasks involving walking throughout the shift. Distances typically less than 200 feet with occasional walking greater distance
- Occasional sitting during duties
- Occasional standing and does not need to be static.
- Occasional squatting and/or bending, 5. Lifting up to 10 lbs. frequently, 11 to 25 lbs. occasionally, 26 to 50 lbs. seldom and lifting greater than 50 lbs. is not required.
- Repetitive movements that may require following proper body mechanics. Notify supervisor of any concerns.
- Occasionally push/pull up to 25 pounds of force.
- Occasional twisting, use of stairs and frequently reach from shoulder height to knees; seldom reach above shoulders or below knees.
- During the Covid-19 outbreak, N-95 masks are to be worn during the duration of your shift. When facility is not in an active outbreak, staff are to wear surgical masks, KN-90/95s unless on observation which N-95 are still mandated.
Salary : $124,000 - $130,000
