The Huron County Board of Developmental Disabilities (HCBDD) is the primary agency for supporting people with developmental disabilities in our county.

Job Overview
The Case Manager will work with a caseload of individuals aged 22 and up.
Responsibilities
· Serve as primary contact for individuals on their caseload.
· Provides Person-centered planning for individuals on their caseload.
· Works with providers and families
· Coordinates and monitors services according to the Individual Service Plan.
· Complete case management documentation according to department requirements.
· Coordinate services with other social service agencies.
· Conduct Reflections/Monitoring as outlined by team; annually complete ISP or as needed, complete assessments annually or as needed.
· Ensure than an emergency response system is available 24 hours per day, 7 days per week. Carry and respond to on-call phone calls on a rotating basis.
· Appropriately balance an individual’s right to privacy and others’ need to know sensitive information about individuals served.
· Complete home visits, provider visits, or visits to other locations to ensure quality service delivery.
· Complete all documentation timely to accurately coordinate and monitor the services provided to individuals.
· Resolve complaints and ensure investigation of allegations of abuse, neglect and/or any other violation of individual rights. Demonstrate appropriate action planning and follow-up skills. Complete follow-up on all UIs and MUIs.
Experience
- A Bachelor's Degree is required.
- A background in social services is preferred.
- Strong communication skills, both verbal and written
- Ability to work independently as well as part of a team.
