What are the responsibilities and job description for the Patient Navigator OUD position at Huntsville Hospital System?
Overview
The OUD (Opium Use Disorder) Patient Navigator helps coordinate care for OUD patients with complex needs, assists with the transition from the hospital to home, and connects patients with necessary community resources to enhance the quality of life. The OUD Patient Navigator works with patients to ensure they understand the discharge instructions, reduce readmissions, reconcile medications, and assists the patient by coaching self-management of disease processes. The OUD Patient Navigator will also provide intervention opportunities to OUD focused resources for SA treatment.
Qualifications
Education: Require BS degree in Social Work or related field
Experience: Two years of Substance Abuse Counseling or Peer Support experience preferred. Comparable combination of education and experience will be considered. Previous substance abuse counseling or hospital based social service experience very beneficial.
Additional Skills/Abilities: OUD Patient Navigators are expected to demonstrate excellent communication skills, to be capable of working with a diverse population and to behave ethically and professionally. OUD Patient Navigator interacts with professionals from many different community based organizations, so they must be able to collaborate well with others.
