Care Coordinator

Overview

Our At Home By Hunt team is growing!  We are seeking an organized, detail orientated Care Coordinator who can demonstrate a customer service approach in each interaction with prospective and current Members.  This position reports to At home By Hunt’s Executive Director, and this will be a dual role with care coordination and assistance with the sales process. Although the main office space is located in Nashua, there is also frequent travel to resident homes and various speaking engagements throughout New Hampshire.

Responsibilities

• Communicate regularly with the At Home By Hunt Executive Director regarding expectations within the department.
• Provide care coordination to Members within the AHBH program when there is a change in condition or an acute medical event.
• Communicate regularly with Members and their families as needed to address issues and assess needs.
• Monitor Members receiving care. This includes oversight and management of care between agencies providing care and the Member receiving care while maintaining Member satisfaction.
• Develop and maintain individual care plans for Members.
• Assist with obtaining appropriate documentation from the home care agencies that are providing services to Members in their homes; request current licensure and insurance information from agency annually.
• On-going relationship building with current and new home care agencies throughout NH, as well as other healthcare institutions.
• Assist Executive Director with the sales process, including connecting with prospective Members through phone calls or other appropriate communication, and maintaining data in CRM.
• Active participation at informational seminars about At Home By Hunt (in-person and virtual) for prospective members on a monthly or as needed basis to successfully grow the membership.
• Represent At Home By Hunt at professional meetings and other networking opportunities.
• Maintain personal/professional growth and development through attendance at seminars, workshops, and through professional affiliations. Maintain professional licensure as appropriate.
• Excellent interpersonal skills with demonstrated ability to interact and communicate effectively with others.

• Experience with older population and challenges they face with aging.

• Discharge planning skills.

• Knowledge of community resources.

• Experience in healthcare setting and strong assessment skills, with the ability to understand healthcare terms and diagnoses.

• Ability to observe changes in health condition proactively.

• Knowledge of care coordination between patients, institutions, and home care.

• Organizational skills and an ability to work well with a team and/or individually depending on the task required. 

• Other tasks as assigned.

Qualifications

• Bachelor’s Degree preferred or equivalent job experience; specifically in Human/Social Services, Nursing, or Certification in Case Management.
• Appropriate licensure for profession (RN, LPN, etc.) as required by State of NH
• Ability to travel throughout NH with primary office space located in Nashua. 
• Sales experience preferred.
• Able to assist members to and from appointments.
• Lift a minimum of 50lbs, bend, sit, repetitive motion.
