What are the responsibilities and job description for the Home Care Coordinator position at Hummingbird Home Care?
Essential Duties and Responsibilities:
- Conduct thorough client assessments to determine individual care needs and create customized care plans.
- Coordinate and oversee the matching of caregivers to clients based on skills, experience, and compatibility.
- Serve as the primary point of contact for new and existing clients to ensure quality service, delivery and satisfaction.
- Develop and maintain relationships with referral sources, including hospitals, rehabilitation centers, senior communities, and healthcare providers.
- Participate in community outreach and marketing activities, representing the agency at local events and professional meetings.
- Perform regular follow-ups and quality assurance visits to monitor service effectiveness and client well-being.
- Collaborate with scheduling and operations teams to ensure continuity of care and efficient communication.
- Maintain accurate documentation and records in compliance with agency policies and regulatory requirements.
- Associate’s or Bachelor’s degree (preferred).
- Administration, Nursing, Social Work, or a related field (preferred).
- Minimum of 2 years of experience in home care, case management, or healthcare coordination.
- Demonstrated ability in marketing, community outreach, or referral development.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently while managing multiple priorities.
- Valid driver’s license and reliable transportation required.
