JOB TITLE: Case Manager / Housing Assistant

Reports To: Director of Programs/Quality Improvement & Director of Property / Housing Services

CLASSIFICATION: Non-Exempt

Position Summary

The Case Manager / Housing Assistant serves as a resource for participants who reside at HMH Permanent Supportive Housing for formerly homeless residents in each community we serve. The objectives of the Case Manager/Housing Assistant include providing on and off-site support thereby improving the daily functioning and permanent housing of all HMHI Residents. Through the development of a client-centered Individualized Service Plan, possible areas of support include mental health, substance abuse recovery, financial literacy, life skills education, self-care and transportation.

Position Responsibilities

Resident Support

  • Implement the mission and vision of HMHI in a manner consistent with its core values
  • Provide daily case management for each individual, complying with all required regulations.
  • Maintain as required appropriate documentation on each individual.
  • Develop Individual Service Plans (ISP) with objective and quantifiable goals that are aligned with each individual’s needs and the HM Housing program standards.
  • Conduct needs assessments, diagnostic assessments, and trauma assessments with participants as required and as able under licensure or certification
  • Guide and advocate for each individual in planning and setting attainable objectives to meet educational, economic, social, emotional, and physical goals.
  • Assess progress towards goals, identify barriers and adjust individual ISPs to address barriers.
  • Coordinate referrals and appointments to local social service agencies including mental health, drug/alcohol treatment, vocational guidance and Ohio Department of Job and Family Services.
  • Provide support, encouragement, and accountability to assist each individual in successfully completing his ISP.
  • Appropriately intervene in crisis situations, balancing the need for safety for all involved. Seek appropriate assistance as needed.
  • Coordinate and facilitate weekly life-skills group meetings.
  • Maintain relationships with partner agencies and local referral sources.
  • Work collaboratively with all other HM Housing personnel.
  • Provides transportation to program participants; transportation may be in agency-owned 15-passenger van or in employee’s personal vehicle.
  • Completes agency-sponsored in person and online trainings as required.
  • Other duties as assigned.

Occupancy Coordination

  • Work toward maintaining full occupancy at assigned locations.
  • Respond to resident requests and concerns within standards and timeframe established by company protocol, maintaining a person-centered approach in communications.
  • Assist all applicants with the related paperwork to process their application and verifications within standards and timeframe established by company protocol
  • Coordinate the complete move-in process to include, but not limited to:

o Resident orientation

o Assisting resident in obtaining needed documentation

o Completing all required paperwork in the physical file

o Entering tenant information into all required databases.

  • Complete database entry of tenant information and work orders within standards and timeframe established by company protocol.
  • Complete the receipt, deposit, and recording of rent payments.
  • Maintain and manage waiting list per company established protocol.
  • Complete the entire annual recertification process, to include scheduling the recertification meeting with the resident and assisting residents in providing necessary documentation.
  • Coordinate the entire move-out procedure, including, but not limited to:

o Collecting keys from resident;

o Obtaining documentation from the resident regarding their move out date;

o Verification of unit vacancy;

o Completing the move out inspection with maintenance technician;

o Coordination of checklist for items needing repair and cleaning;

o Make recommendation regarding the settlement of security deposit;

o Maintain ongoing communication with the maintenance technician with regard to progress in the make-ready of the vacant unit.

  • Complete ongoing apartment inspections in concert with maintenance staff, according to standards and timeframe established by company protocol
  • Other duties as assigned.

Compliance Coordination

  • Ensure all tenant information is accurate in the physical file, OneSite, and other required databases.
  • Completion of government monitoring compliance reports:

o HAP Billing

o TRACS Monitoring

o State, County Funders

o Department of Housing and Urban Development

  • Maintain knowledge of the EIV and HUD Clips website
  • Ensure all certifications are complete and accurate to ensure proper payment is received
  • Monthly posting of charges
  • DEVCO certifications and annual reporting
  • Other duties as assigned

Maintenance Coordination

  • Coordinate maintenance requests and work orders.
  • Implement system for receiving and tracking resident maintenance requests.
  • Complete month-end maintenance reports in database.
  • Conduct ongoing, scheduled unit inspections with maintenance staff, according to standards and timeframe established by company protocol.
  • Identify potential building and/or property issues through ongoing walk-throughs of the interior and exterior site. Notify maintenance staff and complete work orders for any areas needing repair.
  • Other duties as assigned.

Resident Coordination

  • Provide residents with building, program and social information on a regular basis.
  • Communicate with service coordinators regarding resident needs
  • Assist with the social activities if needed
  • Participate in agency fundraising as assigned.
  • Other duties as assigned.

Qualifications-Housing Assistant

  • High school education required.
  • Computer skills to include ability to work in the following basic Microsoft Programs: Outlook, Excel and Word.
  • Must be a person-centered team player, able to balance multiple tasks, open to change and have the ability to learn the landlord management software program.
  • Ability to interact with participants/residents in a manner consistent with the mission and core values of Humility of Mary Housing is required.
  • Must have a valid Ohio driver’s license and reliable transportation

Qualifications-Case Manager

  • Bachelors or Masters Degree in Social Work, Human Services or other related discipline required
  • LSW or LPC Required
  • Familiarity with resources in the community we serve and within the Continuum of Care or other homeless systems required
  • Must have excellent organizational, interpersonal and communication skills
  • Experience with Homelessness, Chemical Dependency and Mental Health Preferred
  • Experience with Critical Time Intervention, Motivational Interviewing, Eviction Prevention, Trauma Informed Care, or De-escalation practices Preferred
  • Must have a valid Ohio drivers license and reliable transportation

Working Conditions

Normal office working conditions. The employee conducts the day-to-day requirements of their job in a typical office environment, utilizing a computer and working at their own desk and other areas of the office. The noise level in the work environment is usually moderate. Willingness and ability to travel to multiple agency facilities and other meetings that may be required.
