Position Title: Outreach Navigator

Reports to: Street Outreach and Supportive Housing Supervisor

Description: The primary goal of the Outreach Navigator is to identify and engage with unsheltered homeless neighbors through direct street outreach activities and through community referrals from mental health, hospitals, local law enforcement and municipalities and other social service agencies. In identifying and engaging highly vulnerable homeless neighbors, the Outreach Navigator will administer assessment tools (with the overall goal of obtaining permanent housing) and facilitate placement into emergency shelter, if needed, and connect to social services and permanent housing.

Qualifications:

Education: Associate’s Degree, or Bachelor’s Degree, with preferred major in social services, or related fields, and minimum of 2 years of experience in social services

Or

High School diploma or GED and minimum of 4 years of experience in social services or case management.

Experience:

Individual must possess demonstrated experience with crisis intervention, Trauma-Informed Care, and social work practice with chronically homeless individuals and/or individuals with disabilities (mental illness, substance abuse/addiction, physical disability, developmental disability, chronic health problems, etc.) Individual must also be capable of preserving the confidentiality of employee and participant records.

Preferred experience includes: working with people who have experienced homelessness or personal experience of homelessness

Responsibilities:

Commitment to Ethical and Effective Services

  • Know and implement HHSI’s mission, policies, and procedures; and abide by all city, state, and federal laws, and regulations.
  • Maintain trainings in suicide risk assessment, mandatory child and dependent adult abuse and neglect reporting, crisis intervention, and civil rights, housing discrimination, harm reduction, trauma informed, housing first practices and street outreach best practices.
  • Regularly evaluate and manage a caseload of unsheltered participants
  • Adhere to HHSI’s agency values in all aspects of work.

Case management:

  • Meet with unsheltered participants within 24 hours of receiving a referral from another organization, public or community partner
  • Assess basic needs and safety of individuals and connect them immediately to emergency services if needed
  • Complete the VI-SPDAT with each individual and facilitate unsheltered participants access onto Coordinated Entry
  • Assist in obtaining housing readiness documentation such as ID, social security card and income verification
  • Connects participants to mainstream benefits and assists them in applying when applicable
  • Provide support to people as they transition from experiencing homelessness to housing
  • Maintain complete client records, daily activity logs, mileage logs, and other reports as directed.
  • Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other local officials), and providers of services and resources to homeless neighbors.
  • Attend team meetings, case conferences, training workshops and community meetings as needed.
  • Provides crisis intervention and advocacy for participants, as needed

Documentation:

  • Completes daily case notes, service transactions, and contacts then uploads them to ServicePoint.
  • Maintains daily log of interactions including location, person and applicable notes
  • Obtains chronic documentation for unsheltered participants
  • Maintains releases of information for all participants and external providers when applicable
  • Completes fund requests for participant needs when applicable
  • Refers participants to other programs at HHS for which they are eligible, along with supportive documentation

Community Outreach:

  • Act as liaison specifically for the local library’s, police departments, private business owners, jails, schools and local street outreach teams
  • Acts as on-call contact for outreach to new people experiencing homelessness, the police department, and other community partners

Other Duties:

  • Collaborates on case plans for high need participants with the Shelter Supervisor
  • Case Conferencing weekly with the Shelter Supervisor
  • Maintains knowledge of top locations of camps and locations frequented by unsheltered individuals and communicates these locations with other community partners conducting street outreach
  • Maintains strong knowledge of community resources, and mainstream resources especially as it relates to housing
  • Regularly promotes the agencies’ mission and advocates for social justice
  • Performs other duties, as needed
  • Performs 80% of job duties outside of regular office setting, including engaging people in campsites, abandoned structures, and other place not meant for human habitation

Schedule:

Full-Time. Must be able to work 36 hours per week. Must have vehicle with valid license, registration and insurance. Scheduled hours will be based on agency and participant needs.

Salary:

Based on qualifications. Travel expenses, and some benefits included.

Lifting up to 50 lbs. is occasionally required. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

The work environment for this position includes a high noise environment, navigating outdoor terrain and scattered-site housing with a variety of accessibility and environmental conditions. Environment includes donated food items, which may contain a variety of allergens. Position also regularly required to use janitorial supplies, such as disinfectants, laundry detergent, air fresheners, etc. Shelter, office, and scattered-site housing may occasionally house service and emotional support animals.

Job Type: Full-time

Pay: $18.10 - $24.37 per hour
