About the Role
We are seeking a Registered Dietitian to join our nutrition team, providing expert guidance and support to our residents, families, and staff. As a Registered Dietitian at Humboldt House, you will be responsible for:
Conducting nutritional assessments and developing individualized nutrition plans
Providing medical nutrition therapy to residents with various dietary needs
Collaborating with our interdisciplinary team to ensure seamless care coordination
Educating residents and families on healthy eating habits and disease management
Maintaining accurate records and documentation of resident care
Responsibilities
Conduct comprehensive nutritional assessments and develop individualized care plans
Implement and monitor resident nutrition plans, making adjustments as needed
Collaborate with our dietary team to ensure meal planning and menu development align with resident needs
Participate in multidisciplinary team meetings to discuss resident care and goals
Stay current with industry developments and best practices in nutrition and dietetics
