As a Home Health Physical Therapist Assistant, you'll work alongside a supervising Physical Therapist to help patients regain strength, improve mobility, reduce pain, and restore independence—all in the comfort of their own homes.
Your encouragement, clinical skills, and compassionate care will help patients achieve goals they once thought were out of reach.

What You'll Do
- Provide skilled physical therapy treatments under the direction of a Physical Therapist
- Implement individualized treatment plans
- Monitor patient progress and communicate findings to the supervising Physical Therapist
- Educate patients and caregivers on therapeutic exercises, mobility techniques, balance improvement, and fall prevention
- Assist patients in improving strength, endurance, coordination, and functional mobility
- Collaborate with physicians, nurses, and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Qualifications
- Active Maryland Physical Therapist Assistant (PTA) License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS knowledge is a plus
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
