As a Home Health LPN, you'll provide skilled nursing care in patients' homes while helping them manage their health, recover safely, and maintain as much independence as possible.

What You'll Do
Working under the direction of a Registered Nurse and according to the patient's established plan of care, you'll:
- Provide skilled nursing visits in patients' homes
- Perform focused nursing assessments and report changes in patient condition
- Administer medications and treatments as ordered and within the LPN scope of practice
- Provide wound care and monitor healing
- Reinforce disease-management and medication education
- Monitor vital signs and patient response to treatment
- Educate patients and caregivers on safety and the established plan of care
- Communicate changes and concerns promptly to the supervising RN and care team
- Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary team
- Complete timely and accurate clinical documentation using WellSky
What You'll Need
- Active Pennsylvania Licensed Practical Nurse (LPN) License Required
- Current CPR/BLS Certification
- Valid Driver's License and Reliable Transportation
- Strong Nursing, Communication, and Documentation Skills
- Ability to Work Independently in Patients' Homes
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
- A genuine commitment to compassionate, patient-centered care
