As a Home Health Speech-Language Pathologist, you'll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while restoring confidence and improving their quality of life in the comfort of their own homes.

What You'll Do
- Complete comprehensive speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled speech therapy services in patients' homes
- Educate patients and caregivers on communication techniques, cognitive strategies, and safe swallowing practices
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Qualifications
- Active Virginia Speech-Language Pathologist License Required
- Certificate of Clinical Competence (CCC-SLP) Preferred
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Communication, and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
