What You’ll Do

Develop and execute strategic marketing plans to grow referrals

Build and maintain relationships with hospitals, physicians, SNFs, assisted living communities, and case managers
-
Identify new referral sources and expand territory reach

Conduct in-person sales calls and community outreach

Track referral activity and maintain CRM documentation

Collaborate with clinical and intake teams to ensure smooth transitions of care

Meet and exceed monthly census growth goals
What We’re Looking For

Minimum 3 years of home health marketing experience (required)

Proven track record of growing census and referral networks

Strong understanding of Medicare home health guidelines

Excellent communication and relationship-building skills
-
Self-motivated, goal-driven mindset

Valid driver’s license & reliable transportation
