Admission Physical Therapist

Prince George's County & Montgomery County, Maryland

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to provide skilled home health therapy services to patients throughout Prince George's County and Montgomery County, Maryland.

• Perform comprehensive physical therapy evaluations in patients' homes

• Develop individualized plans of care based on each patient's needs and goals

• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall functional independence

• Evaluate patients for fall risks and home safety concerns

• Educate patients and caregivers on exercises, mobility techniques, transfer training, and safety

• Recommend appropriate assistive devices and durable medical equipment when needed

• Monitor patient progress and modify treatment plans as appropriate

• Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team

• Complete all required clinical documentation accurately and within required timeframes

• Communicate significant changes in patient condition to the appropriate members of the care team

• Follow agency policies and all applicable state, federal, and Medicare requirements

• Current and unrestricted Physical Therapist license in Maryland

• Graduate of an accredited Physical Therapy program

• Home health experience preferred, but qualified candidates from other clinical settings are encouraged to apply

• Strong clinical assessment and patient-care skills

• Excellent communication, organization, and time-management skills

• Ability to work independently in a field-based environment

• Reliable transportation and valid driver's license

• Ability to travel throughout Prince George's County and Montgomery County
