We have patient needs across the region, but that does not mean you have to cover all three counties.

What You'll Do
- Complete skilled nursing visits in patients' homes
- Perform comprehensive nursing assessments
- Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
- Provide wound care and medication management
- Monitor changes in patient condition
- Provide disease-management education
- Educate patients, families, and caregivers
- Coordinate care with physicians, therapists, and other members of the interdisciplinary team
- Help reduce unnecessary hospitalizations through proactive patient care
- Complete accurate and timely documentation using WellSky
- Active Pennsylvania Registered Nurse License Required
- Graduate of an accredited nursing program
- Valid Driver's License and reliable transportation
- Strong assessment and clinical decision-making skills
- Excellent communication and organizational skills
- Ability to work independently in patients' homes
- Home Health and OASIS Experience Preferred, but Not Required
- WellSky Experience is a Plus
