We are seeking an individual who is interested in serving patients in our Assertive Community Treatment (ACT) Team
Full Time Position 35-40 hours per week
Work Location: Hybrid remote in Caldwell, ID 83605
Responsibilities
- Meet patients in the community and assist with all around service.
- Work closely with the team to ensure that patients are safe and are receiving the services they need.
- Meet with the team to plan and discuss ways to keep patient engagement.
- Act as a bridge between clients and social services, legal systems, and healthcare providers.
- Participate in a 24/7 rotating on-call schedule to provide immediate support when it matters most.

Qualifications:
- Pass a background check.
- Bachelor’s or master’s degree in social work, Psychology, or a related human services field.
- Experience in behavioral health and/or case management is preferred, but if you are new to the field and passionate about helping people, we would love to show you how.
- Strong communication skills and the ability to build rapport with clients.
- Knowledge of the NASW Code of Ethics and an understanding of confidentiality laws.
- Above average attention to detail is needed to stay in compliance with all the documentation requirements.
- Self Drive, a successful case manager is able to manage their time, complete documentation, meet deadlines.
- Ability to navigate Electronic Medical Record systems.
- Valid Drivers License
- Vehicle insurance (minimum coverage for Bodily Injury - 100,000/300,000)
