Human Services of Faribault & Martin Counties seeks a full-time (40 hours a week) Social Worker - Developmental Disabilities Waver Case Manager position. This position is located in our Faribault County Human Services Center (Blue Earth, MN). 


We are looking for a compassionate and organized Developmental Disabilities Waiver Case Manager to join our team and support children and adults with disabilities. In this role, you will conduct MnCHOICES assessments to determine eligibility for long-term support services and develop person-centered plans that promote safety, independence, and quality of life. You will provide ongoing case management for individuals eligible for Rule 185 and Home and Community-Based Services (HCBS) Waivers, coordinating services, monitoring progress, and ensure compliance with DHS guidelines. You will also collaborate with families, caregivers, and community professionals to build strong support networks. This position reports to a Social Services Supervisor and offers the opportunity to make a meaningful impact while working in a supportive, mission-driven environment. The HSFMC comprehensive benefit package includes health insurance (including option for high-deductible health plan) with employer sharing in cost, employer-paid $20,000 term life insurance, earned sick and safe time, generous paid leave policy, and 11 paid holidays each year. As a public employer, we provide a defined benefit annuity through PERA (employer contributes 6.5% plus 1%; employee contributes 6.5%). Employees have options for employee-paid dental, vision, accident, critical care insurance, and additional life insurance. Employees can contribute pre-tax to health savings account (per qualifications), pre-tax flexible spending accounts, short and long-term disability insurance, and Section 457 deferred compensation plans (Corebridge and Nationwide). Office Hours are 8:00 a.m. to 5:00 p.m., Monday – Friday; flexible work hours can often be accommodated to balance family and personal life. Salary range is $30.91 - $42.04 per hour. Starting salary range is $30.91 - $33.72 per hour.

Minnesota Merit System Examples of Duties

• Interviews clients at intake.

• Carries a caseload where a need for continuing social casework predominates.

• Provides counseling to families and individuals.

• Conducts complex case evaluation for the purpose of assessing problems and determining appropriate types and methods of treatment.

• Prepares intensive long or short-term treatment plans, which require a fund of casework knowledge.

• Identifies client situations, which require intensified service and brings to the attention of supervisor for help or referral.

• Provides vocation/employment services.

• Provides direct services to emotionally, physically, or socially handicapped individuals.

• Provides services to delinquent children.

• Provides or assists in providing services to other cases involving difficult or complex social and financial problems. Interprets programs to clients, refers clients to appropriate community services.

• Interprets social and emotional factors to others involved in treatment of clients.

• Prepares social histories with emphasis on psychosocial factors, adoption, or foster home placements.

• Supervises foster home placements.

• Interviews and screens prospective adoptive parents.

• Provides preventive services.

• Interprets policies and regulations to clients.

• Provides casework services for rehabilitation and care of children.

• Prepares and maintains case records which meet federal, state and local guidelines.

• Dictates findings and correspondence.

• Prepares regular and special reports.

• May provide work direction to social work para-professionals.

• Participates in in-service training and other staff development activities to increase knowledge of social work processes and skill in application to individual cases.

Minnesota Merit System Minimum Qualifications

• A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.

• A bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.
