JOB TITLE: Case Manager
DEPARTMENT: Family Services
PROGRAM: Community Family Resource Center
STATUS: Full Time - Non-Exempt
REPORTS TO: Program Manager
HOURS PER WEEK: 40
LOCATION: Bell Gardens, CA
Position Summary
The Case Manager provides family-centered case management services and will be responsible for implementing a multi-faceted family engagement service delivery system designed to strengthen family wellbeing and access to resources. This role serves as a key connector between families, internal staff, and community-based partners by providing case management, needs assessments, referrals, and ongoing support to ensure families receive the services they need to thrive.

Compensation
In accordance with California law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.

Duties And Responsibilities
- Conduct needs assessments with children, families, or individuals to identify strengths, barriers, and priority needs.
- Develop individualized service plans and provide referrals/linkages to community-based services.
- Monitor service plan progress and adjust supports as needed to ensure goals are met; reassess goals as needed.
- Facilitate warm handoffs to partnering agencies.
- Provide case management services that support system navigation and promote self-advocacy.
- Collaborate with internal and external stakeholders to ensure coordinated service delivery.
- Provide outreach services and program presentations in the community.
- Maintain accurate, timely and confidential records.
- Promote equity, community leadership, and representation within program services.
- Communicate effectively through verbal communication, email, and phone calls.
- Attend required meetings, trainings, agency-related activities; represent the organization professionally.
- Demonstrate adaptability and the ability to multi-t
- Perform other related duties, as assigned.
- Knowledge of local community resources, social service systems, and best practices in family engagement.
- Knowledge of mental health issues and other risk factors, highly desirable.
- Competency to work with the various ethnic/cultural groups in the target area/community.
- Must be able/willing to travel as position requires cross collaboration partnerships, trainings, and meetings across LA County.
- Strong relationship-building/maintaining, networking skills, and great organizational skills.
- Proven competency skills, as well as the ability to demonstrate sound judgment and make independent, well-informed decisions.
- Good organizational and time-management skills and ability to work well under pressure.
- Able to communicate effectively orally and in writing.
- Ability to meet attendance standards and work additional hours as necessary, including evening hours.
- Maintain high level of confidentiality.
- Ability to follow program and agency policies and procedures.
- Ability to work effectively in a team setting.
- Preferred Bilingual (English/Spanish) highly desirable.
- Proficient in Microsoft program applications.
- Education: Bachelor’s degree in Social Work, Sociology, Criminal Justice, or related field.
- Experience: Must have at least one year of experience in case management, family engagement, or related community-based services; working with families in provision of direct services to low-income and ethnically diverse clients in a non-profit setting; experience providing services to families affected by mental health issues and other risk factors, highly desirable; and experience/expertise in social media and audio/visual equipment, highly desirable.
- Must reside in Supervisorial District (SD) 4.
- Driving is required on a daily basis. Must provide own vehicle, valid California Driver’s License, and proof of full insurance coverage.
- Negative tuberculosis screening.
- We require the selected candidate to undergo a criminal background check using the Live Scan fingerprinting method in compliance with applicable laws and regulations.

- Work in moderate noise levels and non-smoking environment
- Ability to occasionally lift and/or move up to 25 pounds independently

- Occasionally may be required to stoop, kneel, crouch, or crawl
- Must be able to speak and hear clearly to communicate effectively
- Constantly operates a computer, keyboard, and other various office machinery
- Majority of job duties are performed onsite; however, visits to different sites may be required
