What are the responsibilities and job description for the Case Manager position at Human Services Agency in Suffolk County, NY?
Case Manager needed to provide financial advocacy in non-profit agency’s housing department. This worker will receive financial literacy training in order to foster economic independence skills for participants who reside in emergency shelter, transitional housing and rental assistance programs. The case manager will meet individually with survivors of domestic violence to identify goals, create a plan for independence and secure permanent housing. The case manager will conduct workshops on financial literacy and enroll participants in credit repair program designed specifically for survivors and enroll in employment program. The case manager will be expected to transport participants in agency van.
Preferred:
· Strong Case Management skills
· Knowledge of trauma informed care
· Self-starter
· Proven motivational skills
· Experience working with participants in residential setting
· Experienced with budgets and conducting presentations
Spanish speaking a plus. Bachelor’s degree in Social Work or Human Services preferred. Full time/benefits included. Valid driver’s license required.
