Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. As a Case Manager, you will work closely with clients to assess their needs, develop personalized plans, and connect them with appropriate services. This position requires strong interpersonal skills, empathy, and a commitment to helping others achieve their goals.
Responsibilities
- Conduct comprehensive assessments to identify client needs and challenges.
- Develop individualized service plans in collaboration with clients.
- Provide crisis intervention and support during critical situations.
- Facilitate access to community resources, including Section 8 housing assistance and workforce development programs.
- Offer mentoring and support to clients in navigating social services.
- Monitor client progress and adjust service plans as necessary.
- Maintain accurate documentation of client interactions and case progress.
- Collaborate with multidisciplinary teams to ensure holistic care for clients.
- Provide supervision and guidance to interns or junior staff as needed.
Experience
- BACHELOR'S DEGREE REQUIRED, Social Work or related field. NEW GRADS ARE WELCOME TO APPLY!
- A background in social work or related fields is preferred.
- Experience in crisis management OR behavioral health highly desirable.
- Familiarity with motivational interviewing techniques is a plus.
- Strong communication skills and the ability to build rapport with diverse populations are essential.
