SUMMARY: CSP Providers maintain responsibility coordinating and providing services for adults who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers.

DUTIES:*
- Accept assigned referrals and engages clients to initiate services.
- The CSP Providers assist in obtaining necessary assessment data, including vital signs and participates in integrated treatment coordinating which may include, treatment plan, releases of information, progress notes, and referrals to internal and external providers.
- The CSP Providers arranges meetings with client and other providers and coordinates needed client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs.
- They work cooperatively with other service providers and has a good understanding of program eligibility requirements and referral aspects associated with community resources.
MINIMUM QUALIFICATIONS:
- Bachelor's degree in Human Services or related field and 2000 supervised hours of mental health experience
- OR High School Diploma and 6000 supervised hours of mental health experience.
- Valid Driver's License and reliable vehicle with proof of insurance.
- Ability to submit and pass a DMV/Criminal background check.
- Interpersonal skills and cultural competency are key in this position.
DESIRED QUALIFICATIONS:
- Bachelor's degree
- Two years of related experience.
- Experience working with adults who have been homeless or at risk for homelessness, indigent, have chemical use/abuse issues, and have been involved with legal system.
