What are the responsibilities and job description for the Community Support Program (CSP) Provider - Duluth position at Human Development Center?
Community Support Providers will coordinate and provide services for adults diagnosed with a mental illness. Services may include providing ARMHS (Adult Rehabilitative Mental Health Services), MH-TCM (Mental Health Targeted Case Management), or a combination of both. This position will work closely with all community support services to help clients live more successfully and independently in the community. The successful candidate will work with a multi-disciplinary team of internal and external providers to assess needs and apply interventions appropriately.
