$21.85 - $32.80

Education Required
High School diploma or GED required.
Preferred Education
Bachelor’s degree in a relevant human service field
Certification Required
Valid GA driver’s license required, excellent driving required and have reliable transportation.

- Engages a population of socially vulnerable patients in their care, assist them through the process of working towards better health by providing support, encouragement, and education in person, by telephone and via electronic means.
- Builds relationship with respect to diversity, using active listening, casual counseling to strengthen the patient’s health, social and personal goals.
