Hudson Regional LTC Pharmacy is the premiere provider of long-term care pharmacy services in the Hudson Valley, specializing in the care of the developmentally disabled and others requiring ongoing care.
Job Summary
The Registered Nurse Consultant is integral part of the pharmacy team, acting as a liaison to agencies and performing chart reviews.
Essential Functions
Conducts pharmaceutical chart reviews to determine the reasonableness, appropriateness and medical necessity of medications for the developmentally disabled population
Monitors drug therapy regimens for contraindications,drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
Analyzes laboratory results, reviews consults, reassess and documents necessary recommendations
Provide medication storage audits
Teaches various classes, including AMAP training classes
Functions as a liaison between pharmacy and agencies
Instruct and provide technical support for pharmacy web portal
Administer immunization clinics
Job Requirements and Qualifications
Must be a licensed registered nurse in good standing in the state of New York and possess a valid driver’s license.
Reasoning Ability
Must demonstrate proficiency and productivity in a fast-paced environment with unscheduled interruptions while maintaining acceptable judgment, responsibility, and assumption for decisions, consequences, and results. Applicant must demonstrate good organization, time management, and communication skills. Occasionally must be able to adjusted work schedule as required to accommodate company needs.
Technology Skills
Must demonstrate proficient ability and practical knowledge of personal computer use, word processing software and spreadsheet software. Additionally, the registered nurse consultant must demonstrate timely and proficient use of company specific software programs and any other handheld or technology tools as required for completing job tasks.
