Key Responsibilities
- Assessment & Intervention: Conduct comprehensive psychosocial assessments to identify barriers to care and develop personalized intervention plans.
- Counseling & Advocacy: Provide high-level clinical counseling to patients and families, offering emotional support and crisis intervention during acute medical episodes.
- Care Coordination: Facilitate the movement of patients through the healthcare continuum by collaborating with multidisciplinary teams to ensure efficient and safe discharge planning.
- Regulatory Navigation: Apply expert knowledge of Medicare/Medicaid regulations and JCAHO (Joint Commission) guidelines to ensure all care transitions meet stringent compliance and quality standards.
- Resource Management: Act as a liaison to community resources, ensuring patients have access to the necessary post-acute support systems, including home health, rehabilitation, or outpatient mental health services.
- Education: Educate patients and their support networks on disease management, healthcare rights, and the available community-based social services.
- Education: Bachelor’s degree in Social Work or a related field.
- Experience: 2–4 years of related clinical experience in a healthcare or hospital setting.
- Licensure: Must be a New Jersey Licensed Clinical Social Worker (LCSW).
- Communication: Exceptional ability to communicate clearly in English, both orally and in writing, to facilitate complex discussions between medical staff and families.
- Technical Knowledge: Proven proficiency in discharge planning, JCAHO guidelines for continuity of care, and Medicare/Medicaid regulatory frameworks.
- Preferred Skills: Previous experience in a psychiatric or behavioral health setting is highly preferred and considered a significant plus
Salary : $39 - $55
