Job description
We are currently looking for a Part Time Nurse Practitioner (NP) for Home Health. The home health assessment will be conducted on Medicare Advantage Plan members residing in Henry County, MO. The average home visit will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit.
Responsibilities:
- Conduct annual home health risk assessments for a Medicare Advantage Health Plan
- Provide education on medical conditions and medications.
- Identify any gaps in care and provide recommendations for resolution.
- No creation of treatment plans or prescribing of medications is required.
- Work Hours: Flexible shifts between Monday and Sunday.
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment: 10 hours
- COVID vaccination is REQUIRED.
Experience Needed:
- Minimum 6 months recent experience as Nurse Practitioner REQUIRED
- Previous In-Home care/In-Home health experience REQUIRED (i.e. Hospice/Palliative Care, in-home clinical rotation) REQUIRED.
- Nurse Practitioner License in relevant state (current unencumbered).
- Board Certification through AANP or ANCC.
- Education Requirements: MSN degree from an accredited school of nursing
- Comfortable with independent visits in private home setting
- Must have personal vehicle
Job Type: Part-time
Pay: $80.00 - $85.00 per hour
