MAIN PURPOSE

The Community Health Worker Lead provides support to the Community Wellness Program and serves as the liaison/lead of the Coordinator of Special Programs I Community Health Workers in the coordination of outreach events, programs and initiatives in designated zip codes; collaborates with community partners to ensure community connection to services; and tracks outreach activities and community health outcomes related to reducing health disparities.


This position is eligible for hybrid telework.


POSITION DUTIES

60% Engagement, Outreach, and Communications

  • Maintains liaison and fosters collaborative relationships with a variety of community-based organizations and Health Department partners in identified zip codes to promote CHW/HCHD programs and services
  • Communicates effectively with colleagues, management, and inter-departmental staff, community partners, program participants, and the general public
  • Assists with the development of and provides accessible presentations to community members and/or community organizations; represents HCHD/Community Wellness Program initiatives with community partner programs and outreaches to communities in coordination with Supervisor and other Bureaus
  • Engages residents through one‑on‑one meetings in libraries, community centers, and partner sites; build rapport, assess SDOH needs, and provide appropriate referrals.
  • Conducts follow‑ups to ensure successful connection to services while identifying emerging community needs, barriers, and trends.
  • Supports field‑based initiatives—including community events, outreach/tabling, Wellness Stations, CAP surveys, and Photovoice conversations—to strengthen visibility and resident engagement.


30% Program Coordination

  • Monitors and tracks assigned activities such as compiling reports for documented activities and facilitates outreach efforts for CHWs to ensure equitable community access to HCHD and Howard County programs/services
  • Identifies individual and community health needs to develop and recommend improved standards for programs/services
  • Maintains knowledge of State- and Countywide agencies, facilities and community organizations providing health-related services to Howard County residents
  • Recommends, plans, implements, and evaluates activities and strategies related to HCHD-specific programs and strategic plan objectives
  • Establishes and coordinates logistics for organized virtual, in-person, and/or hybrid meetings and events and education sessions with clearly defined goals, objectives, and action items; agenda and activity development, meeting facilitation; communication; planning and implementation of actions; and provides follow-up to facilitate implementation of identified actions/deliverables
  • Tracks CHW activities, referrals, and outcomes; document community needs; and support evaluation efforts such as CAP surveys and Photovoice impact reporting.
  • Coordinates logistics for outreach events, meetings, and education sessions while maintaining up‑to‑date knowledge of countywide resources.
  • Contributes to planning and implementing program strategies, incorporating community feedback to recommend improvements and strengthen service delivery.
  • Attends and participates in meetings such as HCHD workgroup meetings, LHIC full Coalition and assigned workgroup meetings, Food Council meetings, and other partner meetings, as needed.


10% Other

  • Accepts responsibility for special projects and other duties as assigned
  • Participates in appropriate professional education and training for roles and responsibilities of work assignments, as requested or approved by Supervisor; completes CHW certification within one year of hire
  • Participates in seasonal flu clinics, as approved by Supervisor
  • Serves on affiliated HCHD committees as assigned
  • Possesses a broad understanding of HCHD services and can refer to other programs. Participates in necessary drills and exercises, responds to emergencies as appropriate, keeps abreast of policies and procedures in the program, bureau and department. Completes all required trainings. Participates in quality improvement efforts within the program.


MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.


Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.


Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.


DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess the following:

  • Experience with oral and interpersonal communication skills.
  • Experience with Howard County communities and health system.
  • Possession of a State Community Health Worker (CHW) certification or meets the requirements to obtain this certification.
