POSITION SUMMARY:
The Patient Application Clinic (PAC) Coordinator provides client-centered assistance to Howard Brown Health clients living with HIV who are completing Ryan White B Program applications (ADAP) in the Patient Application Clinic. This role is responsible for assessing clients’ healthcare and medication needs, counseling clients on HIV treatment options and adherence to medications, coordinating referrals for supportive services (case management, mental health, legal, insurance etc.), scheduling follow-up medical and lab appointments, and liaising with clients, medical providers, and other relevant organizations to ensure client access to HIV medications. The Patient Application Clinic Coordinator also manages the daily functioning of the PAC and assists with onboarding new staff and managing programmatic goals and data.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Facilitates communication between clients, medical providers, and other outside agencies (IDPH, pharmaceutical companies, pharmacies, insurance companies etc.) to assist clients with accessing HIV medications and medical care.
- Coordinates the daily functioning of PAC through completing scheduled appointments, arranging supportive service referrals, conducting patient outreach, and performing other administrative tasks as needed.
- Oversees tracking tools to monitor the status of completed submissions, missing documents for incomplete applications, and previous submissions that are expired or in need of renewal.
- Ensures clients’ continued access to HIV medications and medical care by identifying and outreaching to clients at risk of falling out of care or losing their healthcare coverage.
- Oversees all non-case managed HBH clients who have completed ADAP and maintains follow-up regarding ADAP needs.
- Oversees all patient referrals from HBH staff regarding ADAP needs and access to HIV medications.
- Maintains a flexible schedule that allows clients to complete PAC appointments in person at most HBH clinics, remotely by telephone, or electronically via email.
- Develops and maintains required records, reports, and statistical data including tracking ADAP submissions, client contacts, and services and referrals provided to generate reports for department leaders, grant applications, and program funders.
- Identifies program goals and work plans, develops progress measures, and creates standard operating procedures in collaboration with the Program Manager.
- Provides education and counseling on the importance of adherence to HIV medications and treatment plans, using copay cards, and assistance programs to obtain medications.
- Provides referrals to services that assist with transportation, food, housing, vision/dental care, financial assistance, health insurance, and other supportive services.
- Consults and collaborates with a multi-disciplinary team including primary care providers, behavioral health staff, nursing, medical assistants, partner services staff, and others involved in client care.
- Manages a caseload and completes progress notes, enters orders, tracks client health outcomes, and documents all communication with clients within the electronic medical record system.
- Attends and participates in all group consultations, department meetings and trainings, team meetings, individual supervision, and any other required meetings and trainings.
- Assists in the training of volunteers and staff providing services to clients, as needed.
- Performs related duties as assigned.
QUALIFICATIONS, KNOWLEDGE, & SKILLS REQUIRED:
Education:
- Bachelors’ degree in social work or related social services field is required.
Experience:
- 0-2 years of experience working in a community-based medical/social services setting is required.
- Experience working with clients living with HIV and with diverse communities is required.
Knowledge, Skills, and Abilities:
- Ability to maintain the highest level of client confidentiality
- Knowledge of clients living with HIV and diverse communities

- Ability to participates in continuing education, attend all trainings, meetings, and professional development events as requested by supervisors or funders to maintain current knowledge.

- Master’s degree in social work or related social services field is preferred
- Desire to work for and within LGBTQ communities is preferred
Working Conditions:
