Compensation Range: $24/hr - $31/hr commensurate with experience 

 

Benefits:

Overview:

The Senior Case Manager is responsible for case management and supportive care coordination for asylee families/single adults (Men or Women).

 

Responsibilities:

Primary:

  • Provide case management services (conducting psychosocial assessments, developing treatment plans, assisting with obtaining and retrieving vital documents, assisting with applying for benefits and entitlements, performing room inspections, and preparing and gathering documents needed for housing placement) to up to thirty (30) families (and/or Single Adults).
  • Handles complex, high acuity, or specialized caseloads, supports quality assurance and internal compliance, assist supervisors with training and caseload reviews, serves as the lead case manager when supervisors are unavailable.

Secondary:

  • Maintain client records as per funder and regulatory requirements, as well as Housing Works standards.

Other Functions:

  • Provide referrals, and follow-up, when necessary, for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
  • Utilizing the vast array of internal services available to clients, look to connect (and coordinate) with other departments including: ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Help new clients move into their apartments and assist clients with their transition from homelessness to stable housing.
  • In collaboration with the client, develop strength based individualized goals. Provide constructive feedback on goal participation during scheduled engagement.
  • Assess and discuss financial management with the client, including following up with the client on any rent obligation, maintaining an active subsidy, consistently making utility payments, and food and nutritional needs.
  • As needed, accompany the client in the community on scheduled appointments.
  • Actively participate in weekly supervision with your supervisor. Come to the meetings prepared.
  • Complete all required reports and paperwork by the prescribed deadlines.
  • Demonstrate excellent customer service with residents/clients; staff; vendors; and all other relevant stakeholders.
  • Perform other responsibilities and duties as assigned.
  • Enthusiastically take part in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS.

Key Performance Indicators (KPIs)

  1. Ensuring clients have updated ILP's (every 30 days.)
  2. Regular bi-weekly check-ins with clients to monitor and support their progress towards achieving both short-term and long-term goals, including the completion of assessments and Independent Living Plans (ILPs).
  3. Provide support in identifying housing opportunities, including options such as family reunification, voucher-funded housing, and independent living.

 

Requirements:

Education:

  • Bachelor’s degree is required in social work, psychology, counseling, or related field, or two (2) or more years of relevant experience.

Qualifications

  • Must be proficient in Microsoft Outlook and Office (Word, Excel, and PowerPoint).
  • Good communication skills, both verbally and written, including the ability to communicate with all levels of management.
  • Knowledgeable of CARES system
  • Dependable & flexible.
  • Ability to maintain a high level of confidentiality.
  • Understanding of harm-reduction practices.
  • This position requires working in the office 100% of the time.
  • Ability to travel throughout the local NYC community, including utilizing public transportation.
  • Conduct monthly room inspections.
  • Ability to work one weekend-day each month
  • Willingness to work occasional overtime and offer flexibility regarding scheduling work time.
  • Bi-lingual (Spanish) is strongly preferred.
  • Ability to work with the homeless population which may include migrants.

    Competencies

    • Ability to exercise discretion and tact in all interpersonal contacts and always maintain client confidentiality.
    • Strong problem solving and conflict resolution skills
    • Good communication skills both verbally and written including the ability to communicate with all levels of management and a variety of stakeholders.
    • Deep understanding of harm-reduction practices.

    Essential Physical Demands/ Working Conditions

    • Housing Case Manager position requires the staff person to be present in the program site 100% of the time during night hours.
    • Occasionally expected to de-escalate tense, even hostile, client situations.
