The Housing Services Program Manager is responsible for the leadership, coordination, and quality oversight of Housing Visions’ NYS 1115 Medicaid Waiver programming, in partnership with Healthy Alliance. This role oversees case managers delivering Health-Related Social Needs (HRSN) services, including housing stabilization, care navigation, and related supports to eligible Medicaid Managed Care members. The Program Manager ensures compliance with NYSDOH, Social Care Network (SCN) Lead Entity, and Managed Care Organization (MCO) requirements and fosters a trauma-informed, outcomes-driven approach to service delivery. The overall goal of CONNECT is to support residents in securing and maintaining safe, quality, affordable housing and fostering a supportive environment that reflects the mission, vision, and values of Housing Visions.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Oversee the day-to-day implementation of HRSN programming, ensuring high-quality service delivery, accurate reporting, and contract compliance with NYSDOH, SCN Lead Entity, and MCO requirements to meet the strategic goals and objectives of Housing Visions and the CONNECT team.
• Supervise, train, and evaluate case management staff delivering Enhanced HRSN Services, ensuring adherence to documentation protocols and performance standards.
• Ensure timely and accurate use of the SCN IT Platform (Unite Us) to track referrals, screenings, assessments, and Social Care Plans.
• Train and support staff in HRSN domains, including screening, navigation, and trauma-informed practices.
• Ensure HIPAA compliance by enforcing privacy protocols, training staff, and overseeing secure handling of client information across systems and partners.
• Monitor program performance metrics and lead continuous quality improvement efforts, including participation in SCN reviews and implementation of improvement plans.
• Maintain collaborative relationships with SCN partners, including Healthy Alliance, MCOs, and other CBOs.
