The Homelessness Prevention Coordinator for Santa Clara County enables people with developmental and other disabilities and their families to secure short-term rental assistance and remain securely housed while they apply for, qualify for, and move into affordable housing. The Homelessness Prevention Coordinator works actively with community partners to increase housing stability and access for people with developmental and other disabilities and their families.

Intake, Assessment, and Rental Assistance for New Referrals from Community Partners as well as Clients Already Served by Housing Choices

  • Respond to referrals of new clients from community partners in a timely manner, including completing a profile in the Homelessness Management Information System (HMIS), securing the required Release of Information (ROI), and assessing eligibility for short-term rental assistance using the prescribed Homelessness Prevention Assessment Tool (HPAT).
  • Respond to requests for assistance from Housing Choices’ Housing Coordinators or Resident Coordinators supporting existing clients who may be eligible for short-term rental assistance, including completing a profile in the Homelessness Management Information System (HMIS), completing the required Release of Information (ROI), and assessing eligibility for short-term rental assistance using the prescribed Homelessness Prevention Assessment Tool (HPAT).
  • For clients determined to be eligible for financial and rental assistance, assist the client, the client's Housing Coordinator, or Resident Coordinator to assemble all required documentation for submission of an application for short-term rental assistance.
  • For clients awarded short-term rental assistance, create and implement a housing stability plan drawing on all appropriate community resources to help the family remain stably housed. Or support the client's existing Housing Coordinator or Resident Coordinator in creating and implementing a housing stability plan drawing on all appropriate community resources to help the family remain stably housed.
  • Oversee each client’s successful exit from the short-term rental assistance program.

Case Management Practices

  • Maintain detailed case notes on a daily basis and provide current data about the client on the Housing Choices’ Salesforce database and in the HMIS database.
  • Communicate regularly with the client, service coordinators, service providers, community partners, and support persons to mobilize efforts to enable clients to meet housing application requirements or maintain housing when secured.
  • Provide accurate, complete, and timely information on services delivered for purposes of billing and program reporting to third parties.

Position Requirements:

  • Knowledge of all aspects of affordable housing and the recurring housing issues faced by people with developmental disabilities.
  • Active listening skills and commitment to fully understand the client’s point of view.
  • Ability to communicate complex housing choices to clients who possess a range of listening and processing abilities and prior life experiences.
  • Ability to maintain a non-judgmental and non-defensive communication style in a variety of challenging discussions.
  • Outstanding interpersonal skills, including the ability to work with a diverse community and build collaborative relationships with a variety of community stakeholders.
  • Ability to apply critical thinking skills in developing and time-sensitive situations and make sound independent judgments.
  • Ability to prioritize competing tasks and follow through in a timely manner.
  • Ability to follow meticulously the organization’s record-keeping and reporting protocols.
  • Ability to follow directions and work cooperatively as a team member with other staff, as well as with the consumer, family, service coordinator, service providers, and support members.

Physical and Environmental Conditions:

  • Required standing, walking, and sitting, both inside and outside.
  • Ability to engage in extended periods of computer work.
  • Ability to drive to multiple office and residential locations.
  • Ability to sit at a desk for several hours at a time and sit for meetings that may last for several hours.

Minimum Qualifications and Experience:

  • BA or BS required, preferably in Social Services or another relevant field.
  • Preferred candidates have at least two years of relevant experience in providing direct service to low-income individuals (additional relevant education may be substituted).
  • Must possess a valid California Drivers License, proof of automobile insurance, and a clean driving record.
  • Must pass Department of Justice criminal background check.
  • Fully bilingual (written and oral) and bicultural in English and Spanish is a plus.
  • Knowledge of community resources, services, and benefits in the assigned county.

Job Types: Full-time, Part-time

Pay: $28.84 per hour
