Key Responsibilities
- Communicate with patients and families to assess care needs and provide ongoing support
- Develop, coordinate, and manage individualized patient care plans
- Schedule and coordinate appointments and services with healthcare providers
- Educate patients and families on medications, conditions, and care instructions
- Monitor patient progress and maintain accurate documentation
- Collaborate with nurses, aides, and healthcare professionals to ensure quality care delivery
- Maintain professionalism, empathy, and excellent customer service at all times
Qualifications
- Minimum 3 years of experience in care coordination, healthcare administration, or clinical practice
- Bilingual in English and Bengali required
- Strong communication, organizational, and multitasking skills
- Proficiency in Microsoft Office and general computer systems
- Ability to work in a fast-paced healthcare environment
