What are the responsibilities and job description for the Licensed Practical Nurse position at House of Possibilities Inc?
House of Possibilities is seeking a part-time (20 hours/week) Licensed Practical Nurse (LPN). The individual in this position will provide part-time nursing support within HOPe's Community Based Day Supports program, CCAP. The LPN will deliver basic clinical care, medication support, health monitoring, documentation, and member-specific health training. In addition they will support compliance with DDS regulations (115 CMR 5.00).
The position has no supervisory duties.
Some of the essential duties and responsibilities of this position include
- Performing Direct Nursing Care - including monitor health status and report changes, assist with physician-ordered treatments and respond to on-site health incidents.
- Medication & Health Support - Assist with medication administration consistent with MAP rules and LPN scope, maintain logs, counts and documentation, Monitor medication storage, report discrepancies.
- Training - Train staff on individual health support needs with in LPN scope, reinforce MAP protocols.
- Documentation & Compliance - Maintain accurate medication records, support incident reports related to health events, ensure compliance with DDS and agency procedures.
- Collaboration - communicate with RN, Sr. Director, Program Directors, and Case Managers, provide clinical input at IDT meetings when requested, coordinate with families and external providers for routine health updates.
Requirements
- Maintain professionalism and confidentiality.
- Complete all mandatory training.
- Maintain required licensure and certifications.
- Minimum 1 year experience as LPN. DD/IDD experience preferred.
- Competence in basic nursing tasks, medication assistance, and clinical documentation.
- Ability to communicate clearly with staff, families, and providers.
- Familiarity with DDS regulations preferred.
- Basic technology proficiency.
Education/Certifications/Licensure
- Current LPN license in Massachusetts.
- Current First Aid/CPR.
- MAP certification required.
Salary : $32 - $42
