Job Description
JOB TITLE: Navigator I
REPORTS TO: Director of Operations
STATUS: Full-Time, Hourly, 38-40 hours per week
GENERAL SUMMARY:
The Navigator I will assist partners providing navigation/coaching who are currently experiencing homelessness, at risk of homelessness, or economic instability. The Navigator will support partners by utilizing a holistic centered plan.
Assisting with teaching the Foundations and Getting Ahead cohorts, and providing back-up when needed to other
Navigators. The Navigator I will support the Mission, Vision, and Values of the organization while working with the
Director of Operations to facilitate the execution of the current Strategic Plan of HOTEL INC.
Duties include but are not limited to:
Navigation: Assist partners to alleviate impacts of poverty and develop lasting solutions.
- Assist with educational components provided through partnerships and community engagement
- Provide intake for new program partners through integration into Charity Tracker and KY HMIS
- Utilize assessment tools in holistic partnership
- Create individualized holistic plans with program partners while monitoring and evaluating program partner’s weekly, bi-weekly, or monthly progress within Charity Tracker; Work with each partner to address any financial, health, food access/nutrition, education, employment, legal, community or spiritual needs impacting their housing situation
- Determine eligibility for appropriate housing and other supportive service programs both in-house and with community partners and assist partners with obtaining/maintaining permanent housing
- Obtain federal, state and local benefits; assist Program Partners with completing SSI, SSDI, Medicaid,
Medicare RX, SNAP and other entitlement applications as needed
- Assist the partners with obtaining any necessary health services including scheduling medical appointments/referrals, mental health referrals, assistance with obtaining health benefits, etc.
- Assist Program Partners with household management including home inspections, conflict management, supplementing nutrition, and the use of public transportation for any medical care/employment as needed
- Regular participation in holistic plans/case meetings with HOTEL INC’s team of Navigators to objectively examine the progress of all partners engagement and activity
- Provide navigation in the field, as needed, with a Street Medicine Team or during regular business hours with other Navigators (min. 6x per month)
Other Duties:
- Equip self with a working knowledge and eligibility requirements of community resources
- As needed, provide support to people living on the streets in extreme weather and temperatures
- Provide assistance when needed in other areas of HOTEL INC
- Performs other duties as assigned by the Director of Operations.
Minimum Qualifications:
Bachelor’s Degree in Social Work, Public Health, or other related field or 1-2 years field experience
Problem solving skills
Willingness to serve low income individuals and families, and individuals with disabilities, addiction and trauma
Willingness to serve and lead as part of a team, and to attend necessary training
Some travel required
Valid driver’s license
Excellent computer skills: Word, Excel, data entry and email
Excellent oral and writing skills/presentation
Must be able to lift 20-40 pounds
Must pass a drug test and background check
Must be able to work one evening per week
