Advanced Practice Provider (FNP/PA), HSS Spine

Open to 4 or 5 days a week with possible hybrid option

no weekends/holidays

The Advanced Practice Provider (APP) is a Nurse Practitioner or Physician Assistant with training in the nursing model or medical model of care. Hospital for Special Surgery (HSS) APPs are committed to personalized, high-quality healthcare and are held to the same legal and ethical standards of care as Physicians and Surgeons.

The APP will triage and evaluate patients’ needs as they receive treatment for acute or chronic back pain. In this role, the APP will work closely with Spine Surgeons at the forefront of their fields, partnering with integrated, highly specialized clinical care teams to advance HSS Spine’s mission of delivering the highest-quality care for spine patients that is both comprehensive and patient-centered.

As the first clinical touchpoint for HSS Spine patients, the APP will play an instrumental role in providing a full range of operative and non-operative care for HSS Spine patients. Reporting to the Chief Administrator, Spine Service, the Care Coordinator will support programs across multiple sites.

Responsibilities:

  • Partnering with all HSS Spine access points (i.e., HSS Connect, internal referrals, HSS Spine Center with Stamford Health, Ambulatory Care Center, etc.) to triage patients to the right physician, whether it be a Spine Surgeon or non-operative Physician (e.g., Neurologist, Physiatrist, Pain Management, etc.), and develop an initial care plan
  • Diagnosing and treating acute spine conditions
  • Ordering, performing, and interpreting diagnostic tests such as lab work, x-rays, and metabolic bone evaluations
  • Ordering and performing pre-surgical workups
  • Prescribing medications and other treatments
  • Referring patients to non-operative care options as indicated
  • Managing patients’ overall care
  • Educating patients on disease prevention and positive health and lifestyle choices

Qualifications

Requirements:

  • Master’s or doctoral degree from an accredited physician assistant school or school of nursing
  • Licensure as an FNP and RN or PA in the state of NY
  • Board certification as an FNP or PA
  • DEA
  • CPR certification: BLS
  • 2 years of PA/NP experience; experience in managing patients with low back pain preferred
  • Ability to prioritize issues, resolve conflicts and communicate with all levels of the organization
  • Experience working with and motivating patients to achieve personal care goals
  • Team oriented and energized by serving as a major contributor to scaling care management offerings for ambulatory patients at HSS
  • Must possess ability to learn new software tools & systems as required on the job
  • Self-motivated, detail-oriented, and pragmatic; energized by working in a fast-paced, evolving environment

Desired Qualifications/Skills:

  • Proficiency with Epic and Microsoft Office Suite (Excel, Outlook, PowerPoint, Word, etc.)
