Responsibilities:
- Provide comprehensive nursing care to patients in a variety of settings, including hospitals, clinics, and homes.
- Assess patient needs and develop individualized care plans.
- Coordinate and collaborate with healthcare team members to ensure optimal patient outcomes.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Educate patients and their families on disease management, treatment options, and preventive care.
- Utilize case management skills to coordinate healthcare services and resources for patients.
- Conduct utilization management activities to ensure appropriate use of healthcare resources.
- Document patient information accurately and timely in electronic health records.
Qualifications:
- Valid Registered Nurse (RN) license in the state of GA/SC.
- Minimum of 2 years of nursing experience, preferably in Hospice.
- Knowledge of pediatric nursing principles and practices.
- Strong patient service orientation with excellent communication skills.
- Solid understanding of human anatomy, physiology, and pathophysiology.
- Experience in case management or utilization management is highly desirable.
- Familiarity with critical care and ICU settings is a plus.
- Ability to assess vital signs and interpret diagnostic test results.
- Proficient in utilizing electronic health record systems.
- Strong organizational skills with the ability to prioritize and multitask effectively.
