Job Summary
The Community Liaison builds and maintains relationships with community referral sources, healthcare and human services professionals, and senior‑focused organizations to promote the PACE Georgia program and support enrollment growth. This role increases program visibility through targeted outreach, collaborates with operations to align community engagement with organizational goals, provides information to prospective participants and families, and ensures timely documentation of referral activity in the CRM
Qualifications:Minimum Qualifications
- Bachelor’s degree in related field
- One year of previous healthcare experience that includes interaction with community members
- Valid driver's license, automobile Insurance and reliable transportation required.

- Previous sales or marketing experience preferred
- Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension
- Excellent organizational skills

- Understanding of community resources, human services networks, senior‑serving organizations, and healthcare referral pathways.
- Proficiency in entering, maintaining, and interpreting referral and outreach data within a Customer Relationship Management platform
- Ability to identify barriers to referral or enrollment and work collaboratively to address them
- Comfortable adjusting outreach strategies, schedules, or messaging in response to community needs or organizational priorities.
Physical Demands and Work Environment
This position requires the ability to sit, stand, and walk for extended periods throughout the workday. The role involves frequent use of the hands and wrists, including fine motor tasks such as keyboarding, pinching, and other repetitive motions. The employee may occasionally be required to carry objects weighing up to 10 pounds and may need to lift up to 10 pounds from floor to waist level or from waist level to above. The position also requires adequate sensory abilities, including near and far visual acuity, color discrimination, depth perception, and hearing, in order to perform essential job functions safely and effectively. Position Type and Expected Hours of Work
This is a full‑time position. The standard work schedule is Monday through Friday, 8:30-5pm for a total of approximately 40 hours per week. Additional hours may be required based on operational needs.

Must be able to travel as required including between different facilities and community locations.
