Job Summary:
The Social Worker in a home health setting provides psychosocial support and counseling to patients and their families, helping them navigate the emotional, social, and environmental challenges associated with illness, disability, and recovery.
Working as part of an interdisciplinary care team, the MSW assesses patients' social and emotional needs, develops individualized care plans, and connects patients with community resources to promote optimal health outcomes and independence at home.

Requirements
Education/Qualification
- Master's degree in social work
- One year of social work experience in a mental health or medical setting is required
- Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation
Key Responsibilities:
Delivery of Care
- Completes comprehensive psychosocial assessments, providing counseling and crisis intervention, assisting with advance care planning, and connecting patients to community resources including but not limited to financial, housing and transportation support
- Works in collaboration with the interdisciplinary team and home health director
- Develops and carries out individualized plans of care related to the social determinants of health
- Identifies appropriate resources to meet patient/caregiver/family needs
- Serves as an advocate for patients and families
Interdisciplinary Team Process
- Actively participates with the interdisciplinary group (IDT)
- Works effectively with other IDT members with a focus on collaboration in problem-solving patient needs and working with the patients' plan of care
- Provides effective report to the IDT based on regulatory requirements for patient care planning evaluation
- Maintains effective relationships and partnerships with community healthcare providers, agencies, and senior services
Documentation
- Completes required documentation according to agency accreditation standards
- Ensures that notes and patient records are complete, accurate, current and clear
- Documents consultation and/or referrals to Home Health leaders for clinically complex cases requiring advanced clinical interventions
