POSITION SUMMARY

Schedule: varies - days, nights, and weekends.

Under the direction of a Registered Nurse, the Hospice Licensed Vocational Nurse provides skilled nursing for assigned hospice patients. The Hospice LVN provides skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, provides supportive care to the patient and family, and participates in the care plan implementation.

QUALIFICATIONS

1. High school Diploma or GED.

2. Graduate of an accredited school of vocational/practical nursing.

3. Current license within the State, required. Certification in Hospice and Palliative Vocational/Practical Nursing, preferred.

4. Minimum of one year experience as a LVN/LPN in a hospice setting.

5. Ideal candidate has a minimum of one year experience with Netsmart software program.

6. Demonstrated competency in performance of skilled nursing functions.

7. Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.

8. Ability to maintain a high level of confidentiality concerning patient and family information.

9. Competent using computers with knowledge of electronic medical records documentation.

10. Applicants need to reside in our service area - see below. This position requires a personal vehicle in order to perform the essential job functions. A valid California driver’s license including an acceptable driving record, and auto insurance coverage, as set forth below, are required. You will be required to travel within the organization’s area of service which includes; Aguanga, Canyon Lake, Fallbrook, Hemet, Homeland, Lake Elsinore, Menifee, Murrieta, Nuevo, Perris, Quail Valley, Romoland, San Jacinto, Sun City, Temecula, Wildomar, and Winchester. This travel may include freeway, city and rural road travel and travel during busy commuter times.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following non-inclusive list. Other duties may be assigned. All duties are to be performed in compliance with applicable laws, regulations, and Hospice of the Valleys policies and procedures.

1. Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.

2. Implements current nursing practice following a comprehensive plan of care.

3. Provide accurate and timely documentation of patient services to reflect plan of care.

4. Obtain report from RN prior to making patient visit.

5. Report changes in the patient’s conditions and needs to the RN Case Manager, Triage RN or Nurse Manager.

6. Administers prescribed treatments and medications according to physician/nurse practitioner orders.

7. Provide patient and family/caregiver education and information pertinent to the diagnosis and plan of care.

8. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary team (IDT).

9. Uses and prepares equipment and supplies effectively, adhering to aseptic technique.

10. Provides appropriate pain/symptom management. Observes and reports patient’s response to prescribed treatments/medications.

11. Demonstrate flexibility in patient scheduling and readily accepts new assignments.

12. Observe for safety hazards in the patient’s and organization’s environments and takes appropriate steps to reduce or eliminate them.

13. Participates in organization’s quality assessment performance improvement activities.

14. Maintain continuing education through academic studies, seminars, and workshops.

15. Attend and participate in in-services, staff meetings and case conferences.

16. Complete all documentation in an accurate, legible manner and in the timeframe required by standards and regulations.

17. Performs other duties as assigned by the RN.

18. Coordinate with Triage Supervisor on ordering, stocking, and tracking of all clinical supplies when applicable.
