What are the responsibilities and job description for the Chaplain position at HOSPICE OF THE PANHANDLE, INC?
The Chaplain provides spiritual support to patients/families, staff, and volunteers.
Level of Responsibility: Reports to Social Services Manager
Status: Non-Exempt
Schedule: Monday-Friday, 8 am - 4:30 pm; occasional weekend and evening activities
Service Area: Morgan and Hampshire counties of West Virginia
Duties and Responsibilities:
1. *Assesses on an ongoing basis the spiritual and religious needs and resources of patients and families.
2. *Identifies and addresses signs and symptoms of spiritual distress.
3. *Implements and coordinates the provision of spiritual care to alleviate spiritual distress and existential loneliness.
4. *Provides direct spiritual support and/or spiritual counseling to patients/families in accordance with their beliefs.
5. *Demonstrates professional counseling skills encompassing, but not limited to, active listening, reflecting, clarifying, affirming strengths, normalizing emotions, maintaining
healthy interpersonal boundaries, setting goals and limits, involving patients in the plan of care, utilizing empathy as a therapeutic tool, achieving closure in therapeutic relationships.
6. *Establishes and maintains therapeutic relationships with patients and families from a variety of cultural, ethnic, religious/spiritual, and socioeconomic backgrounds.
7. When working in a facility, complies with facility-specific requests for reporting, documentation, PPE use, and other regulatory and facility requirements.
8. *Collaborates with clergy or appropriate spiritual representatives of patients/families to ensure spiritual support. Establishes networks with faith communities throughout the
service area to teach hospice philosophy and enlisting aid in providing spiritual support.
9. *Collaborates with the interdisciplinary team in evaluation of and updates to the plan for spiritual care of the patient/family unit.
10. *Develops and executes a plan of care that accomplishes the goals and objectives of the patient and family, and advocates for the spiritual autonomy of the patient when appropriate and necessary.
11. *Documents according to agency policy/protocol.
12. *Provides spiritual support to staff and volunteers as needed to support their role with hospice if requested.
13. Performs occasional liturgical assignments.
14. *Conducts or makes arrangements for funeral or memorial service as needed.
15. *Provides education programs for community clergy, religious and lay representatives, staff and volunteers.
16. Provides educational opportunities for professional students assigned.
17. *Complies with the employee standards.
18. *Meets visit average standards established by the organization.
19. Other duties as assigned.
