Job Type: Part-Time
Schedule: After-Hours / Monday - Friday 5:00pm to 10:00pm - This is an on-site position based in Carlsbad. The role includes occasional field visits to patients' homes on an as-needed (PRN) basis to provide skilled nursing care. This position is also eligible for a $2.00 per hour shift differential for eligible hours worked. What You'll Do
- Provide skilled nursing visits for palliative care patients in their homes.
- Assess patient needs and report changes in condition to the Registered Nurse.
- Administer medications and treatments within LVN scope of practice.
- Provide education to patients and caregivers regarding disease progression, symptom management, medications, and care techniques.
- Respond to after-hours patient needs, triage calls, and urgent symptom management visits.
- Collaborate with physicians, nurses, social workers, and the interdisciplinary team to ensure continuity of care.
- Complete accurate and timely documentation in the electronic medical record.
- Support quality improvement initiatives, chart audits, documentation review, intake activities, and clinical operations as assigned.
- Graduate of an accredited Vocational Nursing program.
- Current California LVN license in good standing.
- Minimum one year of LVN experience (hospice, home health, palliative care, or medical-surgical experience preferred).
- Excellent assessment, communication, and critical thinking skills.
- Compassionate, patient-focused approach to care.
- Comfortable working independently while collaborating with a multidisciplinary team.
- Valid California driver's license, reliable transportation, and current auto insurance.
- Hospice and Palliative LVN Certification (preferred but not required).
- Experience with electronic medical records (EMR).
- Previous hospice, palliative care, or home health experience.
