Apply today!POSITION: Hospice Clinical Liaison - Full-Time (Monday - Friday, 8:30am - 5:00pm)POSITION SUMMARY:The Hospice Clinical Liaison serves as a vital link between Hospice of the North Coast (HNC) and the community, blending clinical expertise with outreach and relationship-building skills. This role is ideal for a licensed RN, LVN, LCSW, or MSW who brings both healthcare experience and a passion for connecting with others. The Clinical Liaison is responsible for increasing hospice admissions through strategic referral development, education, and community engagement.
