Summary of Duties and Responsibilities: Reporting directly to the Executive Director (ED), The Director of Social Services provides leadership and oversight for the Social Work, Spiritual Care, and Bereavement departments. This position ensures that emotional, psychosocial, and spiritual support services are delivered with excellence, compassion, and consistency across all Foothills Compassionate Care (FCC) programs. Working closely with the interdisciplinary team, this leader upholds our mission to provide holistic care that honors each person’s dignity, values, and journey.

Reporting Relationship: Executive Director (ED)
Position Type: Full-Time, Exempt, Benefited
Essential Functions:
- Lead, mentor, and support social workers, spiritual care counselors, and bereavement staff and volunteers to deliver exceptional, person-centered care.
- Develop and maintain departmental policies, procedures, and standards of practice consistent with agency goals and regulatory requirements.
- Oversee bereavement programming, including family follow-up, community support groups, and memorial events.
- Collaborate with clinical and administrative leaders to integrate psychosocial and spiritual care throughout patient and family services.
- Support staff education, supervision, and professional growth.
- Participate in strategic planning, quality improvement, and program development initiatives.
- Manage departmental budgets and staffing to ensure efficient, sustainable operations.
- Participates/contributes in ongoing interdisciplinary conversations
- Develops and/or evaluates the Plan of Care (POC) and documents in the EMR
- Serves as Administrator on Call as assigned
- Participate in all FCC meetings as requested
- Participates in QAPI program and FCC sponsored in-service
- Participates in FCC orientation and in-service programs for staff and volunteers as requested
- Supports Patient Volunteer training as well as reviewing applicable training materials and onboarding
- Serve as a resource for complex psychosocial and ethical care issues.
- Performs other duties as assigned by the ED
- Represent Foothills Compassionate Care in community outreach and partnership activities related to emotional, spiritual, and bereavement support
Knowledge and Skills
Required:
- Proven ability to lead / supervise
- Independent judgement to make sound decisions
- Maintenance of confidentiality, HIPAA and patient and employee privacy laws
- Ability to Critically think and provide necessary interventions
- Ability to Communicate effectively, both orally and written
- Demonstrated ability to work effectively with diverse populations and interdisciplinary teams
Desired
- Public speaking and relationship building capabilities with partners, groups, and businesses
Education & Experience
Required
- Masters degree in Social Work, Psychology, Counseling, or a related field required
- Working knowledge of Electronic Medical Records
- 2 years of Hospice, Palliative or healthcare social services experience
Desired
- Current state licensure in a relevant field (LCSW, LMFT, LPCC, or equivalent)
- 2 years of supervisory experience
- Strong knowledge of state and Medicare regulations
License / Certifications
Required
- Current CPR certificate
- Valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance

Pay: $95,680.00 - $108,160.00 per year
