Our mission is to significantly enhance the end-of-life experience by ensuring quality hospice care provided in homes, facilities, and in our Inpatient Care Center, a state-of-the-art, 12-bed inpatient facility that is the only one of its kind in our area.
We currently have an opening for an experienced RN Case Manager to join our team. The RN Case Manager is responsible for ensuring the highest quality of care for our hospice patients in their place of residence. The ability to coordinate care in a variety of settings including acute care facilities, nursing facilities, assisted livings, and home settings is essential for this role. Qualifications:
- Graduate of an accredited School of Nursing
- Current licensed Registered Nurse (RN) able to work in Texas and Arkansas
- One to two years of nursing experience in acute care, home health or hospice setting
- Hospice experience helpful, but not required
- Work Remotely No
